Health Heros of America will be hosting a Flu Vaccination clinic for Kent Students and Staff on Monday, Sept. 27, 2021. The nurses from Health Heros will bring flu shots to our district.
Students/Staff will receive the vaccine during school hours. The timing of visits to each building will be determined by the number of consent forms received. Family members of students and staff, as well as staff who can not make the building vaccine times, are welcome to submit a form to receive the Flu vaccine at Roosevelt on Monday evening, from 3pm - 5pm. Anyone over the age of 4 years is eligible to receive the Flu Vaccine.
Your consent form must be turned into the school's clinic office by Thursday, Sept. 23, 2021 by 2:00pm in order to be eligible to receive a vaccine on Monday. Printed forms are available on the pick up table outside the main office or you can print one from the link here.
If you do not turn in a consent form you will not be able to receive the Flu vaccine.
